Advocacy, Disabled/Handicapped available at Goodwill Industries Easter Seals of Kansas

A support and advocacy service that connects people with
disabilities to resources within the community. Targeted
case management provides support to eligible persons by
developing, linking, coordinating, and monitoring services,
supports, and resources. Individual case managers will
assist with funding resources, applications, and community
supports such as:

Information and referral
Assessments
Person Centered Support Service Planning (PCSP) and coordination
Finding and securing financial assistance for necessary services and/or technologies
Facilitating community inclusion
Discharge planning
